Cohort 2 Learning Session

During this event, you will receive an overview of SHIP and the Public Health District role, improve your knowledge of the Regional Collaboratives and Medical-Health Neighborhood concept, as well as review some key components of your participation including identifying your PCMH team, understanding adaptive and technical changes, developing and documenting your SHIP PCMH Transformation plan, and hear from a panel of cohort 1 participants about their experience.
