Healthcare is inherently fragmented. Mental health has been separated out from other facets of healthcare delivery like primary care for decades. Now, in service to a more integrated approach to delivery, practices are beginning to come together in service to providing more coordinated care for their community. However, this shift does not come without a few challenges. Barriers often include information exchange between providers and the tracking and following of referrals between clinics. This interactive workshop will focus on strategies to best connect primary care to mental health with practical lessons for application.
